Inspections are on hold at this time due to the recent passage of Louisville Metro ordinances relating to Shelters and Transitional Housing. We will post info as it becomes available.
Fill out and submit the form below to request certification as a transitional housing provider for the Louisville Metro area. After receiving your request, we will contact you to start the process and send you an application form. Payment is due with the completed application form. Your efforts in becoming a certified provider will assist the community and those using your services. Thank you.
Initial Registration Fee: $75.00 (One time charge for first location operated)
Additional Registration Fee: $50.00 (For each additional location operated. Added to Initial Fee)
Renewal Fee: $50.00 (For each location operated. Must have been certified the previous year)
Each location used for residential placement must be registered and inspected in order to be listed as a certified and approved transitional housing location. Certification must be renewed every 12 months.
If your agency has a contract with the Kentucky Department of Corrections to provide housing or is certified by The Coalition for the Homeless, then we will honor those as approved housing locations. Please contact us to have your agency listed on our certified housing list. There will be no fees charged since you meet their standards and have their approval. Please advise your status in the Additional Information block to allow us to verify your information.
